*ASSUMPTION OF RISK AND RELEASE OF LIABILITY AGREEMENT** By attending the Death at Dinner event hosted by Death at Dinner LLC, the undersigned Guest acknowledges that participation involves inherent risks including physical injury, illness, allergic reactions, property damage, and alcohol related incidents.  Guest voluntarily assumes all such risks, known and unknown, and agrees that the Company, its officers, employees, agents, contractors, and Death at Dinner LLC shall not be liable for any adverse reactions from food allergens, injuries from slips, trips, falls, or use of premises and furniture, or for any consequences arising from alcohol consumption including intoxication, impairment, accidents, or self harm.  To the fullest extent permitted by law, Guest hereby releases, waives, and discharges the Company and Death at Dinner LLC from any and all claims, demands, liabilities, and causes of action of any kind arising out of or related to participation in the event. Guest affirms they have read and understand this Agreement and sign it voluntarily, and that it is binding upon their heirs, assigns, and representatives.
